Apple Tree Sponsorship Form

Tree sponsorships are $500.00 per tree.  Payment can be made via PayPal or check.  Please include a note with your payment indicating it is for a tree sponsorship. 

Checks should be made payable to The North Carolina Living Historical Farm Committee Inc. and sent to: The North Carolina Living Historical Farm Committee Inc., 308 Horne Creek Farm Rd., Pinnacle, NC 27043 

Please complete a separate Tree Sponsorship Form for each tree you wish to sponsor. Sponsorships for specific trees are limited and will be awarded based on the order in which donations are received.  Select the name of the tree you want to sponsor from the list attached to the previous page. If you would like to list a second or third tree choice in case your first choice is taken, enter the name of the tree below.

Tree Type

Select the type of tree you wish to sponsor. Semi-dwarf trees will grow to approximately 12 to 16 feet tall. Espalier trees are trained to grow to a specific shape and are supported by a trellis system. Espalier trees will grow to approximately 6 to 8 feet tall.
Sponsorship Information

Sponsorships may be made to honor a person or group. Individuals, groups, organizations, or companies may also wish to sponsor a tree. Each sponsorship will be recognized by a 4"x6" plaque at the tree indicating the name of the variety, its place of origin, and sponsor or honoree. Please select the type of sponsorship you would like indicated on the plaque.
